This testing program is for asymptomatic individuals. If you have a fever or symptoms, you will need to complete the Illness Reporting Form and indicate in the notes that you have been selected for surveillance testing at www.fairmontstate.edu/reportillness. You cannot test if you have symptoms.
If you are in quarantine or isolation, you should not leave to visit the testing center. If you have been directed to quarantine, you should complete the Illness Reporting Form and indicate in the notes that you have been selected for surveillance testing at www.fairmontstate.edu/reportillness.
If you have had a positive COVID-19 test within the past 90 days, you should not be tested. Students should notify the COVID Student Liaison that they have been selected for surveillance testing and have had a positive test by emailing covidstudenthealth@fairmontstate.edu. Employees should inform Human Resources of their recent positive test by emailing hr@fairmontstate.edu. If the appropriate documentation is not provided, testing will be required.
